Passport Documents Required (Complete List)
You need identity proof, address proof, and date of birth proof to apply for a passport. Aadhaar Card is the most convenient — it works as all three. Make sure all documents have the same name and details.
Required Documents
Identity Proof (any one)
- Aadhaar Card (recommended — works as identity, address, and DOB proof)
- Voter ID (Election Card)
- PAN Card
- Driving Licence
- Ration Card with photo
Address Proof (any one)
- Aadhaar Card
- Voter ID
- Electricity / Water / Gas bill (not older than 3 months)
- Bank account statement (not older than 3 months)
- Registered rental agreement
- Employer letter on company letterhead
- Post office passbook with address
Date of Birth Proof (any one)
- Birth Certificate (mandatory for applicants born on or after 01-Oct-2023)
- Aadhaar Card
- Matriculation (10th) Certificate
- Driving Licence
- Voter ID
For Non-ECR Category (if 10th pass or above)
- 10th class marksheet or higher education certificate
- Professional degree certificate (Engineering, Medical, CA, etc.)
Additionally Required
- Passport-size photograph — 3.5 cm x 4.5 cm, color, white background
- Self-attested photocopies of all documents
- Original documents (for verification at PSK)
For Minor Applicants (below 18 years)
- Birth Certificate (mandatory)
- Both parents' passports (if available)
- Parent's identity proof and address proof
- Annexure D — declaration by parent (if only one parent is applying)
- School leaving certificate or bonafide certificate (if applicable)
- Court custody order (in case of divorce/separation)
Additional Documents (Specific Cases)
- NOC + Identity Certificate (Annexure B) + forwarding letter — for government employees
- Marriage certificate — if adding spouse name
- Gazette notification — for legal name change
- Court order — for adoption or custody cases
- Old passport (original) — for reissue applications

Tips to Avoid Document Rejection
- Self-attest every photocopy — sign at the bottom with your full name and date
- Carry ORIGINAL documents to PSK — photocopies alone are not accepted
- e-Aadhaar printout is NOT accepted at PSK — carry the physical Aadhaar card
- Name on all documents must match exactly — even a middle name mismatch can cause rejection
- Photo must have white background, no shadows, face covering 80-85% of frame
- For address proof, bills must not be older than 3 months
- If you are a government employee, get NOC and Annexure B from your department before applying
