Fields to fill for each:

  • System – select Grade/CGPA or Division
  • Subject – select your subject/stream
  • If Division selected – enter obtained marks and total marks, percentage will be auto-calculated
  • Score / Percentage – enter exactly as per marksheet
  • Board / University Name – enter full official name
  • Passed Year – as mentioned in marksheet

Tick the declaration checkbox confirming percentage accuracy

💡Fill only if you selected Yes for serving in MP Government / Municipal body

If applicable, fill:

  • Post Held – your official designation
  • Employer's Office Address – full department name and location
  • Service Period From / Till – exact dates (DD/MM/YYYY)
  • Continued in Service / Reason for not being in service
  • Experience Certificate Number – if issued by department

If not in government service, leave this section blank

💡You must enter your own bank account. Payment will not be accepted from someone else's account

Enter the following:

  • Bank Name – full official name
  • IFSC Code – check from passbook or bank website
  • Account Number – double-check before proceeding
  • Name of Account Holder – must match your application name

Upload scanned copy of first page of passbook (JPG, 20 KB to 200 KB)

Do not write your name in the address field

Enter complete address clearly

State, District, Pin Code must match correctly
